Our Primary Care Network (PCN) is called Phoenix Health and shares Health Care Professionals to enable a range of services to be offered to our patients. 

Reception or your clinican may offer you these services if they think it is appropriate but if you are interested in any of them please ask.

Social Prescribing

Many things affect your physical health and mental wellbeing.

Sometimes people need a little help in dealing with the things life throws at them and often they don't know where to turn for help and support. Social prescribing is the process of linking individuals with people, groups or activities in the local area that can help them make some positive steps in overcoming the challenges that they are facing improving their happiness, health and wellbeing.

Health and Wellbeing Coach

A Health and Wellbeing coach supports patients in achieving their health goals and improving their overall wellbeing. They provide personalised guidance, motivation and help patients make sustainable lifestyle changes. Please contact reception to book an appointment.

First Contact Physiotherapist

Our First Contact Physiotherapist can help patients with musculoskeletal issues such as back, neck and joint pain by:

  • assessing and diagnosing issues
  • giving expert advice on how best to manage their conditions
  • referring them onto specialist services if necessary.

Pharmacy Team

Our Pharmacy Team can help you with your medicines, they carry out medication reviews and they help manage our patients with high blood pressure.

Mental Health Practitioner

Our Mental Health Practitioner supports our patients with mental health conditions. They can:

  • Signpost to other support services
  • Give advice
  • Review patients condition
  • Refer to mental health teams and services

Our Paramedics help support our care homes and provide a home visit service when requested by the doctors.
